Major Duties The Louis A. Johnson VA Medical Center is seeking a registered nurse for the Braxton County Community Based Outpatient Clinic (CBOC). As a Registered Nurse, the incumbent utilizes the nursing process and evidence-based practice to collaborate with Primary Care and Mental Health Team members in developing the patient-driven mental health plan of care. The RN is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patient's mental health needs. The incumbent provides patient and family education with a focus on self-management, prevention, and wellness, based on the patient's goals. The incumbent demonstrates leadership by serving as an advocate for patients and a team player to colleagues while continuing to enhance the treatment team's professional growth, development, and practice. Essential to this function is the ability to provide appropriate documentation in a timely fashion, while incorporating advanced clinic assessment concepts and keeping in mind the patient's needs, patient/team flow in delivering care in the most appropriate setting. Daily practice will include triaging, providing initial and ongoing mental and psychosocial assessment; planning and coordinating care when patients are pending discharge from community and VA inpatient services; providing patient and family health education; initiating appropriate intervention protocols; proficiency in verbal de-escalation skills, completing clinical reminders, responding to view alerts; and participating with the specialty mental health team in formulating an interdisciplinary plan of care and documenting assessments, interventions, and referrals in the computerized patient record system (CPRS). Duties include, but are not limited to:
Major duty will be swabbing Veterans that present for testing. Maintains responsibility for quality patient care; demonstrates willingness to support change for the improvement of patient care and unit operation. Provides skilled nursing care and gives guidance to clients in the management of their physical and psychosocial needs. Provides safe, therapeutic environment that protects patient rights. Performs clinical assessment, treatment procedures and clinical reminders posted in the Computerized Patient Record System (CPRS), and other assessment and treatment procedures (e.g., immunizations, injections. Assess client's health status by gathering information and identify actual and potential problems; maintain ongoing assessment status based on information gathered by interviewing and observation. Develop a therapeutic relationship with assigned clients to coordinate psychosocial treatment with standardized physical nursing care and guidance. Develop care plans using assessment data that integrates medical and nursing therapies including input from client's family and other health care team members. Collaborate with other health care team members in delivery of health care services according to established standards and patient care priorities. Monitor client behavior to intervene when appropriate; monitor client for critical changes and initiates appropriate emergency procedures. Maintain a safe environment for clients and strive to meet maximum security and comfort needs. Provide effective management of crisis in disturbed clients in emergency situations. Promote, participate, and evaluate patient/family health education. Incorporate results of quality assurance programs in daily patient care activities. Other duties as assigned. Tour of Duty:

The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. The work requires regular and recurring bending, lifting, stooping, stretching, lifting and repositioning patients or similar activities. The incumbent will be required to lift up to 50 pounds occasionally and up to 35 pounds frequently. The incumbent must be able to transport and transfer patients (e.g. stretcher to wheelchair, bed to stretcher, wheelchair to bed, etc.) safely. The incumbent will be required to handle equipment and must be knowledgeable of an able to use safe lifting techniques. The use of correct body mechanics required. The incumbent should also have the ability to identify the potential for safety hazards. The mental/sensory requirements include recall, reasoning, problem-solving, hearing (aid permitted), speaking clearly, writing legibly, keyboarding, reading, and logical thinking. Also, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13 to 16. The environment's pace can vary from steady to a sometimes fast pace, requiring handling of multiple priorities, frequent, sometimes intense customer interactions, and the ability to adapt to frequent changes in a sometimes noisy environment. There may be the need to use personal protective equipment to prevent exposure to disease, illness and hazardous materials (1-2 hours at a time).

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
